About ACTT

Assertive Community Treatment Team (ACTT) is an evidence-based, intensive community treatment model designed for adults with severe and persistent mental illness who may struggle with traditional outpatient services and are at increased risk for hospitalization, homelessness, crisis episodes, substance use, or incarceration.

Often described as a “hospital without walls,” ACTT delivers comprehensive behavioral health services directly in the community, including homes, shelters, hospitals, workplaces, and other community settings.

Position Summary

The ACTT Qualified Professional provides intensive, person-centered behavioral health interventions, crisis support, rehabilitation services, and care coordination in community and field-based settings.

The QP works closely with psychiatrists, nurses, therapists, peer support staff, and substance use professionals to support members with symptom management, independent living skills, medication adherence, housing stability, employment goals, and community integration.

Services are delivered in accordance with NC DHHS, NC Medicaid, CARF, HIPAA, and ACTT fidelity standards.

Team Collaboration & Daily Operations

ACTT operates using a highly collaborative team-based model focused on communication, flexibility, and shared responsibility.

Each morning, team members participate in structured ACTT meetings to:

• Review member needs and clinical concerns

• Prioritize daily visits and crisis response

• Coordinate schedules and staff assignments

• Develop engagement and intervention strategies

• Ensure continuity of care across the team

ACTT uses a shared caseload approach, meaning staff work together to provide responsive, high-quality support based on each member’s changing needs.

Essential Duties & Responsibilities

• Provide intensive community-based behavioral health services to ACTT members.

• Conduct assessments, interventions, rehabilitation services, and crisis response in home, office, hospital, and community settings.

• Support members with:

• Symptom management

• Medication adherence

• Independent living skills

• Housing and employment goals

• Social and community integration

• Develop and monitor Person-Centered Plans (PCPs).

• Utilize motivational interviewing and assertive engagement techniques.

• Participate in rotating on-call and crisis response responsibilities.

• Coordinate services with providers, hospitals, families, and community resources.

• Complete accurate and timely EHR documentation.

• Maintain compliance with Medicaid, DHHS, CARF, and agency standards.

Qualifications

Required

• Bachelor’s degree in Psychology, Social Work, Counseling, Human Services, or related field

• Must meet NC DHHS Qualified Professional (QP) requirements

• Minimum two (2) years of experience working with individuals with mental health or co-occurring disorders

• Valid driver’s license, reliable transportation, and current automobile insurance

Preferred

• ACTT, CST, IIH, or community-based behavioral health experience

• Experience working with severe and persistent mental illness

• Preferred credentials:

• LCSW / LCSWA

• LCMHC / LCMHCA

• LCAS / LCAS-A

As a key member of our care team, the Intake Coordinator supports individuals and families as they take the first step toward meaningful services. Essential Duties

• Complete all intake and entrance documentation, providing clear explanations of services, forms, and consent requirements to clients and guardians prior to assessment.

• Verify insurance coverage, eligibility, enrollment status, and copays (including BCBS and other payers), and distribute accurate information to appropriate staff.

• Apply for IPRS funding for uninsured clients and maintain complete, accurate, and timely supporting documentation.

• Confirm or create MCO enrollment as needed to ensure uninterrupted access to services.

• Maintain Access to Care (ATC) tracking by accurately entering and updating all referrals and intakes on a daily basis.

• Manage service authorizations within the EMR, including adding units, uploading official authorization letters, and maintaining authorization-related documentation.

• Upload and maintain all required client documentation in the EMR in accordance with confidentiality, safety, and compliance standards.

• Assist with monthly audits and 30-day record reviews, ensuring assigned client records are complete by the 5th of the following month.

• Communicate effectively with clinical providers, physicians, staff, and community stakeholders while maintaining professional conduct and sensitivity to cultural, developmental, and age-related needs.

• Provide administrative support as assigned, including daily lunch coverage and other duties as needed, while adhering to company policies and procedures.

  
  

Education, Training, and Qualifications

• Bachelor’s degree in a Human Services or related health field required, with experience working with the population served; candidates with a non–Human Services bachelor’s degree must possess additional qualifying experience as outlined below.

• Minimum experience requirements include two (2) years of full-time post-bachelor experience for candidates with a Human Services degree, or four (4) years of full-time experience for candidates with a non–Human Services degree.

• Demonstrated working knowledge of organizational policies, mission, and objectives, or the ability to acquire this knowledge within a reasonable onboarding period.

• Completion of all required company onboarding and compliance training within 30 days of hire, including 20 hours of service-definition–specific training within 90 days of hire.

• No substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry.

• Disclosure of any criminal convictions is required; employment decisions will be made in accordance with applicable laws and based on the relevance of the offense to the position.

About the Role

Peer Support Specialists provide one-on-one and group recovery services in homes, community settings, and other approved service locations. Services focus on helping members develop the skills, confidence, and support systems needed to live successfully in their communities.

This position works under the supervision of a Qualified Professional (QP) and collaborates with therapists, medical providers, care managers, and community partners to support each member's Person-Centered Plan.

As a Qualified Professional on our Intensive In-Home (IIH) team, you will deliver direct therapeutic and skill-building services to children, adolescents, and their families — right in the environments where they live and grow. Overview

A Tailored Care Manager serves as the primary coordinator and advocate for Medicaid members with serious mental health disorders and substance use disorders. The TCM works to ensure behavioral health treatment, medical care, housing supports, transportation, benefits, and community resources function together as one integrated care plan.

This position provides whole-person, community-based care coordination for individuals with complex behavioral health needs and high-acuity service involvement.

Key Responsibilities

• Develop and maintain person-centered care plans
• Coordinate behavioral health services including outpatient therapy, psychiatric care, substance use treatment, MAT services, Peer Support, ACTT, CST, and residential services

Overview

A Tailored Care Manager serves as the primary coordinator and advocate for Medicaid members with serious mental health disorders and substance use disorders. The TCM works to ensure behavioral health treatment, medical care, housing supports, transportation, benefits, and community resources function together as one integrated care plan.

This position provides whole-person, community-based care coordination for individuals with complex behavioral health needs and high-acuity service involvement.

Key Responsibilities

• Develop and maintain person-centered care plans
• Coordinate behavioral health services including outpatient therapy, psychiatric care, substance use treatment, MAT services, Peer Support, ACTT, CST, and residential services
• Monitor member engagement and treatment participation
• Identify barriers to treatment and service gaps
• Support transitions following psychiatric hospitalization, detoxification, or residential treatment

• Assist members with accessing Medicaid-covered services
• Help individuals navigate healthcare systems and community resources
• Support applications for benefits, housing resources, transportation, and social services
• Advocate for medically necessary services and continuity of care

• Conduct outreach for members at risk of disengagement or crisis
• Coordinate crisis response services and follow-up care
• Monitor high-risk members to reduce emergency department visits and inpatient admissions
• Participate in discharge planning and post-hospitalization follow-up

• Coordinate communication among providers, family members, hospitals, therapists, psychiatrists, social services agencies, and community supports
• Participate in treatment team meetings, staffing meetings, and interdisciplinary coordination efforts
• Maintain collaborative relationships with internal and external providers

• Complete Medicaid-required documentation and assessments
• Maintain accurate progress notes and care management records
• Track quality measures and care plan updates
• Ensure compliance with NC Medicaid Tailored Care Management requirements

Position Summary

The OPT Assessor is responsible for conducting comprehensive behavioral health assessments and making clinical recommendations for appropriate levels of care and services. This role works closely with consumers, families, referral sources, care teams, and leadership to support timely access to services and regulatory compliance.

The OPT Assessor must maintain strong documentation standards, demonstrate clinical professionalism, and ensure all assessments meet NC Medicaid, Tailored Plan, and organizational requirements.

Essential Responsibilities

• Complete comprehensive behavioral health assessments and screenings

• Conduct clinical interviews and gather psychosocial history information

• Determine medical necessity and recommend appropriate services

• Complete and submit all required documentation within required timelines

• Maintain compliance with NC DHHS service definitions and Medicaid requirements

Position Overview
Pathways To Life, Inc. is seeking motivated and compassionate Behavioral Health Interns to join our clinical team. This internship provides hands-on experience in community-based mental health services and is designed for students pursuing degrees in Social Work, Counseling, Psychology, or related human services fields.

Interns will gain direct exposure to behavioral health practices, documentation standards, and multidisciplinary team collaboration while working alongside experienced professionals in programs such as ACTT, CST, IIH, and TCM.

Key Responsibilities

• Assist with providing community-based behavioral health services under supervision

• Observe and participate in client interactions, including skill-building and support services

• Support development and implementation of Person-Centered Plans

• Complete required documentation in accordance with Medicaid and agency standards

• Participate in team meetings, case staffing, and supervision sessions

• Assist with care coordination and linkage to community resources

• Maintain professionalism, confidentiality, and ethical standards at all times

Learning Objectives

SUMMARY OF POSITION 

The Personal Care Program Director provides leadership, oversight, and coordination for all aspects of the Personal Care Services (PCS) Program. This position is responsible for developing, implementing, and maintaining program operations to ensure compliance with state, Medicaid, and company standards. The Director will work collaboratively with Executive Leadership, Billing, HR, Credentialing, and Operations to ensure program effectiveness, quality service delivery, community visibility, and sustainable growth across all locations.

ESSENTIAL DUTIES & RESPONSIBILITIES 

Program Development & Operations 

Direct and coordinate all aspects of the Personal Care Program, including program structure, policies, procedures, and service delivery workflows 

Ensure the program remains compliant with all applicable state regulations, Medicaid requirements, and internal policies 

Establish and monitor program goals, benchmarks, and performance outcomes 

Oversee the daily management of program operations, ensuring consistency, accuracy, and timely service delivery 

Quality & Compliance 

Maintain ongoing quality assurance reviews and performance monitoring systems
Ensure all documentation meets state and payer standards 

Address compliance issues promptly and implement corrective actions as needed
Maintain readiness for audits, monitoring visits, and regulatory reviews

Staff Oversight & Training 

Recruit, hire, and supervise Personal Care staff 

Develop and implement staff training and development plans 

Provide coaching, mentorship, and performance feedback to ensure staff meet expectations 

Promote teamwork, accountability, and high standards of care delivery 

Billing & Revenue Coordination 

Work closely with the Billing Department to ensure accurate documentation and billing compliance 

Monitor service utilization, productivity, and reimbursement trends 

Support processes that enhance revenue integrity and minimize claim denials 

Community Engagement & Program Marketing 

Actively market the Personal Care Services Program 

Build relationships with referral sources, case managers, discharge planners, and community partners 

        Conduct outreach activities to promote program awareness and drive service referrals 

Represent the program at internal and external meetings, community events, and partner engagements 

Collaboration & Communication 

Collaborate with HR, Credentialing, Billing, and Operations to ensure smooth coordination of all program-related functions 

Provide regular updates and performance reports to Executive Leadership 

Develop relationships with payers, regulatory agencies, and referral sources to support program growth and sustainability

REQUIRED QUALIFICATIONS 

Minimum 3 years of leadership or supervisory experience in Personal Care, PCS, home care, or related human services field 

Strong knowledge of Medicaid regulations, PCS service definitions, and documentation requirements 

Proven ability to manage staff, operations, and compliance simultaneously
Excellent organizational, communication, and leadership skills  

PREFERRED QUALIFICATIONS
      Experience in developing or expanding PCS or home care programs
      Background in compliance, billing, or auditing within healthcare services
      Strong analytical and problem-solving abilities 

WORK SCHEDULE

Monday – Friday, 9:00 AM – 5:00 PM 

Evenings or weekends may be required based on program needs

What you will do

As a CST Qualified Professional, you will be responsible for providing direct care and coordinating services for individuals with mental health and substance use challenges. Working under the supervision of the CST Team Lead, you will play a key role in assessing client needs, developing treatment plans, and ensuring that all services are delivered effectively and in accordance with best practices.

Your responsibilities will include:

• Conducting comprehensive assessments of clients to identify strengths, needs, and goals.

• Developing, implementing, and monitoring individualized treatment plans in collaboration with the client, family, and CST team members.

• Providing direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention when necessary.

• Coordinating and linking clients to community resources, including housing, employment, education, and healthcare services.

• Advocating for clients within various systems, such as healthcare, judicial, and social services, to ensure they receive the support they need.

• Monitoring client progress and adjusting treatment plans as needed to ensure positive outcomes.

• Maintaining accurate and timely documentation of all services provided in accordance with agency policies and regulatory requirements.

• Collaborating with other CST team members, including the Team Lead, Peer Support Specialists, and other professionals, to ensure a holistic and integrated approach to care.

• Participating in regular team meetings and case conferences to discuss client progress and coordinate care.

• Promoting the values of recovery, person-centered care, and cultural competence in all interactions with clients and team members.
